MINNEAPOLIS 19 PETITION
You have two (2) choices for completing your Petition for Membership to Minneapolis Lodge No. 19.ONLINE PETITION
Click on the ONLINE PETITION box to complete your Petition for Membership online. The information you submit within the form will not be saved or forwarded to the Lodge after the "submit" button is pressed. After you hit the "submit" button, the form will be sent to your default printer arrangement.
The petition will then need to be signed by you, two (2) Brothers from the Lodge, as well as, be accompanied by the required fees before it can be accepted by Minneapolis 19. DOWNLOAD PETITION
Click on the DOWNLOAD PETITION box (at left) if you wish to download a PDF version (requires Adobe Acrobat) of the Petition for Membership. You will then be able to complete the information by hand and submit it to the Lodge.
As with the Online method, you will be required to have it signed by you, two (2) Brothers from the Lodge, as well as, be accompanied by the required fees before it can be accepted and voted on by the Lodge.
